Protecting Client Confidentiality

Renascent believes client confidentiality is vital for recovery. We adhere to privacy legislation and have a privacy policy outlining our promise to the client.

Unless we have the client’s written permission or consent, we will not share any information with anyone other than the healthcare professionals who are directly involved in treatment. Renascent will not confirm or deny any connection the client has with us unless we have written permission.

The client can give permission to Renascent to share information by completing this consent form. Consent, however, is never granted to an organization or group; instead specific individuals are granted access to personal health care information. In some cases it makes sense for a number of individuals to be involved, but separate consents must be signed.
